Compiling Documents

Just for fun, I thought I'd let you know some of what we are going through to get started in the adoption process. I knew there would be a lot of paperwork, but I didn't anticipate this much so soon.  Here are just some of the documents that we are in the process of signing, filling out, and gathering. We have to secure all of this before we schedule our home study.

  • Post Adoption Agreement
  • Responsible Individual’s List
  • Home Study Fee and Post Placement Report Fees
  • Autobiographies for Each Prospective Parent
  • Birth Certificates on all Adults and Children residing in the home
  • Marriage Certificate
  • Copy of Driver’s License and Social Security Card
  • Copies of Your Verification of Employment (Employment Letter) 
  • Previous years W-2’s, (months worth of paycheck stubs) from all employers and Current Income Tax Return (copies acceptable)
  • Signed Guardianship Statement 
  • Copy of Financial Statement (Statement of Net Worth) Homestudy Financial Information form
  • International Disclaimer 
  • Education History of Adoptive Parents 
  • Work History of Adoptive Parents 
  • Medical History Form on each member of the household
  • Medical Reports (Physical Examinations) for All Adult and Children Living in the Home 
  • SBI Clearances (Originals required)
  •  Sworn Disclosure Statements for Each Applicant and Adult in the Home.
  • Credit Report (proof of good standing)
  • America World Reading Agreement 
  • Provide 2 (two) relative references (3) non relative
  • Home study Agency Agreement
  • Copy of Vaccination Records for Any Pets
  • Letter from Physician for any past surgeries, diseases, mental illness, current prescription medication, etc
We keep reminding ourselves: One day soon we'll look back on all this and say, "It was well worth it." Thanks for praying for us.
